The neuralgia of trigeminal nerve

The trigeminal nerve neuralgia (trigeminal neuralgia) is characterized by acute excruciating pain along the affected branch of the trigeminal nerve on the face. This disease occurs in 50 people out of 10,000, and more often it affects women over 40. Causes of trigeminal neuralgia Inflammation of the trigeminal nerve (neuralgia) is a pluricausal disease. Hypothermia, catarrhal diseases, and diseases of the upper respiratory tract (sinusitis), herpes infection, facial trauma, unhealthy teeth (caries, pulpitis), and even occlusion abnormality can contribute to neuralgia. Many patients first undergo long-term treatment at the dentist, because they think that bad teeth are the cause of pain, many “lose” completely healthy teeth on the affected side of the face. Some scientists believe that the gradual wear and destruction of the trigeminal myelin sheath plays a role in the disease pathogenesis. Also the great importance plays the width of the anatomical holes through which the branches of the trigeminal nerve come out to the face, with the formation of the so-called “tunnel” syndromes. Psycho-emotional disturbances, nervous stresses, and mental trauma can be provoking factors. Most often trigeminal neuralgia is right-sided. Symptoms of trigeminal neuralgia An attack of pain in trigeminal neuralgia can occur at any time: as a reaction to hot or cold food, to loud sounds, too bright light, even in the process of brushing your teeth. The pain may occur when yawning, shaving, scratching the face, with a smile, from a slight gust of wind. Localization of pain depends on which the affection of the trigeminal nerve branch. The attack lasts 15-20 seconds, and sometimes – up to 2 minutes. Quite often the attack is accompanied by hyperlacrymation and salivation. The pain is more often one-sided, patients compare it with an electrical discharge, so painful and acute it is. At the height of an attack, the facial musculature twitching is observed. In addition, on the face there are so-called “trigger” zones, even a weak touch to which causes an acute neuralgia attack. These “trigger” zones are most often localized on the nose wings and tip, gums and upper lip. The attack begins with the appearance of “prodromes” – the itch of the face skin, sensation of “crawling,” and then occurs in the form of sharp, lighting pain, which lasts no more than couple of minutes. In an attempt to wait out a seizure, a person freezes at the place where he/she was caught by pain. At the same time, patient answers questions in a monosyllabic way, with difficulty opening his mouth, involuntarily smack his lips. In the cold season, seizures occur more often, which become more frequent with time, and their intensity increases. Pain in trigeminal neuralgia is considered one of the strongest pains.
