Documents Required for the Pediatric Ward

An initial consultation with a pediatric neurologist is the first and most important step on the path to your child’s recovery. Having the necessary documents will allow the doctor to make a diagnosis and select the most effective treatment.

Before your visit, you should review the list of required documents and learn how to prepare for the appointment.

What do I need to bring to my first appointment?

Before your first visit to the doctor, it’s important to prepare a specific list of documents and test results. This will help the doctor better understand your situation and provide quality care.

Prepare and bring the following with you:

Birth Certificate

Or the child's passport (for teenagers).

Passport of one of the parents or guardians

A document confirming your identity as an accompanying person.

Medical Records

Excerpts from previous consultations, reports from other specialists, outpatient records—anything that might be helpful to the doctor.

Survey results when available

Such as:

  • MRI / CT
  • EEG (electroencephalogram)
  • Neurosonography
  • Ultrasound, lab test, X-ray, and other results.

What happens during the initial consultation?

During the appointment, the doctor:

Conducts an examination and assessment of the child

Reviewing your medical records

Makes a diagnosis

Determines the next steps in diagnosis and treatment

What do you need to know before your appointment?

The consultation lasts between 30 and 60 minutes.

The consultation may include functional tests or other testing.

Appointments are made only in the presence of a parent or legal guardian.
